Just a year after starting MMA, Manhoef entered the kickboxing ring. He faced Samir Benazzouz in the debut kickboxing match of his professional career on March 16, 1996, at Thai/Kickboxing Nijmegen. However, he was knocked out in round three of the battle. He fought six more fights before going to K-1 World GP, winning three of them and losing three.

He defeated Tatsufumi Tomihira, Rustan Karaev, Paul Slowinski, and Ramazan Ramazanov in a four-bout match at the K-1 World GP. In the twenty-one years he competed in kickboxing, he got 38 wins and 14 losses.

Bellator MMA announced on July 21 that they had signed Melvin Manhoef and Paul Daley. At Bellator 125 on September 19, Manhoef faced off against Doug Marshall in his Bellator debut. In round one, he was victorious by way of KO. He lost to Joe Schilling in his subsequent match on November 15.

At Bellator 133 on February 13, Manhoef faced Alexander Shlemenko to kick off 2015. He was knocked out in round two, ending the fight. After Shlemenko tested positive for anabolic steroids a month later, the outcome was altered to a no contest.

Manhoef lost a contentious split decision in their middleweight championship fight at Bellator 155 on May 20, 2016, against Rafael Carvalho.
